The second failure.

Technical failure and institutional failure. Most of what the shop pays for is the second one.

Bill Gurley said disasters usually have two failures. The technical failure is what happened. The institutional failure is why the people let that become a weak point.

That fits this trade.

The technical failure is the thing that went wrong on the job. The U/G building sewer that missed its invert. The equipment that arrived damaged or was ordered incorrectly. The schedule that slipped. Somebody on the job saw it. Somebody fixed it. Somebody paid for it. The lesson was learned on that job.

The institutional failure is what happens after. The lesson does not move up to the shop. It does not get written down in a way the next job can use. It does not get connected to the next estimate, the next schedule, or the next crew. The job closes. People move on. A different PM and a different crew pay for the same lesson again.

Owners say some version of the same thing. We already learned that one. Then the next job pays for it again.

The gap is not the knowledge.

Most of the failure library is institutional. The knowledge is already in the shop. It is in the daily reports, the job cost, the emails, the texts, and the head of the PM who ran the work. When that person leaves, the lessons that were never written down leave with them.

The gap is not the knowledge. The knowledge is already there.

The gap is getting it from one job to the next.

How Documint PM closes it.

That is the gap Documint PM is built to close.

It sits next to the systems the shop already runs. It reads from them. It does not change them. Job history goes in. What matters comes out. Every insight traces back to the job records. The owner or PM makes the call.

The technical lesson gets fixed on the job. The institutional lesson gets captured so the next job does not pay for it again.

Every job makes the next one smarter. That only happens if the lesson moves up to the shop.
